NW Bergen Regional Health Commission

Our Mission is to provide professional and efficient public health and environmental services to the residents of  the communities we serve. 


Through the delivery of state mandated “Public Health Practice Standards of Performance for Local Boards of Health in New Jersey” and  the policies of the local Board of Health, we protect and promote the public’s health by using health promotion and education, disease prevention, environmental enforcement, and inspection activities.

Local Health Outreach Coordinator (LHOC)

NWBRHC is seeking to fill a position for a full-time Local Health Outreach Coordinator. This position is supported through a grant awarded by the New Jersey Department of Health (DOH). The Local Health Outreach Coordinator will increase local public health capacity to investigate, trace, contain, mitigate and prevent COVID-19 and other communicable diseases through targeted outreach and education to disproportionally affected populations and service entities. The LHOC will have generalized disease investigation and educational outreach responsibilities and will work under the direction of the Health Officer and in collaboration with the Public Health Nurse and Health Educator to assess, plan, coordinate, and implement programming and resources to mitigate and prevent COVID-19 /Other Infectious Diseases in our communities.

Education and Experience: Graduation from an accredited college or university with a Bachelor’s degree in Health Education, Public Health, Community Health Education, Nursing, or a related Health Education field. Certification as a Certified Health Education Specialist (CHES) a plus.

Other Qualifications:

  • Ability to work with diverse professional partners from various community sectors

  • Knowledge of public health and/or community wellness resources and social services in New Jersey

  • Excellent interpersonal skills and ability to interact professionally with people from diverse cultural, racial, ethnic, gender, and socioeconomic backgrounds.

  • Ability to rapidly learn, access, and navigate various databases and software programs following training

  • Strong organizational and time management skills and the ability to manage multiple projects simultaneously.

  • Ability to handle confidential information with discretion and professionalism

  • Excellent written and verbal communication skills and attention to detail.

  • Valid New Jersey Drivers’ license required. The applicant must make available his/her own personal vehicle for the performance of the job to be reimbursed for the expenses of the same at IRS rates.

POSITION OVERVIEW: Conducts communicable disease investigations in our contract municipalities and provides public health nursing services in homes, clinics, and schools to individuals and families in the community. Promotes and protects the health of populations through the performance of core public health functions and the delivery of essential public health services. Performs related work as required.


NUMBER OF WORK HOURS: 20 hrs. per week


COMPENSATION: $35,000 to $45,000 per year.



• Conducts communicable disease investigations using the Communicable Disease Surveillance System (CDRSS).

• Measures patients’ blood pressure and interviews patient to conduct a general health assessment at blood pressure.

• Responds to health questions and monitors disease outbreaks from schools and long-term care facilities.

• Assists at Child Health Conferences (when needed).

• Conducts school immunization audits.

• Administers immunizations at influenza programs.

• Visit homes of individuals and/or families in the community to evaluate health needs and environmental hazards in the home and to conduct childhood lead poisoning assessments.

• Compiles monthly nurse report for the full-time Public Health Nurse.

• Participates in emergency preparedness and response activities.

• Other relevant duties as assigned.


EDUCATION: Registered Nurse License



• One year of experience in a public health field or nursing experience gained while working under the close supervision of a licensed public health nurse.



• Applicants must possess a current and valid registration as a Professional Nurse in the State of New Jersey.

• Appointees will be required to possess a driver’s license valid in New Jersey



• Ability to use Microsoft Word, CDRSS and Lead Trax

• Must be physically able to perform the essential functions of the job, i.e. lift light equipment approximately weighing 10 pounds.
